3、我對在連續(xù)而激烈的比賽中可能發(fā)生的危險已有充分的認識，我保證本人的身體健康狀況能夠承受本次比賽，并具備近期健康檢查證明書。
4、我保證：本人在比賽中因受傷、遭遇意外事故或發(fā)生疾病時，對受到的急救處理方法及結(jié)果不提出異議。
5、我本人、我的家屬、親戚和保護人已充分理解本次比賽的內(nèi)容和意義，并認可我本人參賽。在比賽之前我已作了人身保險。因此，我保證：我自己、親屬及遺囑執(zhí)行人、財產(chǎn)繼承人，對本人在比賽中或參加由本次比賽委會組織的與比賽有關(guān)的活動時，不論什么原因所致的傷、病、亡或出現(xiàn)的后遺癥等，不對比賽組委會提出任何起訴。所造成的損失由保險公司和本人承擔，比賽組委會不再提供因此而造成的損失費。
